Job Description
RN - Case Manager
Looking for RN with Hospice experience within a patient home and facility setting. Pinellas county - assignment based on zip codes within a team's territory No on call or weekend visits Caseload size 17-20 depending on if assigned to a home or facility team Minimum 20 patient visits each week HomeCare HomeBase
- Start Date: 11/2/2026
- Shift Time: 8a-5p
- Contract Duration: 7 Weeks
- Pay: $45
- Requirements:
- Minimum of one (1) year of related experience and/or training. OR -Equivalent combination of education and experience.
- Preferred Skills & Experience: -Prefer one (1) year experience in an acute or post-acute care setting.
- Preferred experience caring for terminal patients and others with life-threatening illnesses.
- Ability to type 25 wpm is highly preferred.
